An increase in T wave alternans is thought to be associated with an increased incidence of cardiac arrhythmias. This study tested the hypothesis that the post-exercise peak in T wave amplitude (TWA) is related to changes in cardiac autonomic activity by comparing these changes in heart transplant subjects versus a control group of kidney transplant subjects. In heart transplant subjects the resting heart rate was higher and the post-exercise peak in TWA was less than half that observed in the kidney transplant subjects. Cardiac autonomic activity contributes to the mechanism of the post-exercise peak in T wave amplitude.
